Preventing and Managing Frailty in Stoke-on-Trent Conference

This conference will bring together people working in health, social care and community settings to explore our approach to preventing and managing frailty, and to promoting independence for older aged adults across Stoke-on-Trent.

The aims of the day include:

  • To ensure the local health system has a model that enables people in Stoke-on-Trent who are moderately and severely frail to maintain independence at home, and when the need arises, to have access to the best possible choice of step-up and step-down care.
  • To hear good practice examples of frailty management and prevention from other Integrated Care Systems
  • To understand the population health management approach and our local population data
  • To test and further develop the proposed local model
  • To confirm the road map required for a new model to be in place for April 2026

This event is for you if:

  • You work in health and social care settings in Stoke-on-Trent
  • You work with older aged adults in the city
  • You are interested in helping us to shape how we prevent and manage frailty across of Stoke-on-Trent

What to expect:

During the day, there will be presentations from a variety of speakers, and opportunities to get involved in group discussions and workshops. A detailed conference pack will be shared with you a week prior to the event. Light refreshments will be provided (tea, coffee), and there will be a 45-minute break for lunch (not provided, but there are a range of cafes and shops nearby).

This conference has been planned collectively with representatives from across the health and care system within Stoke-on-Trent, including NHS Providers, Stoke-on-Trent City Council, Staffordshire and Stoke-on-Trent Integrated Care Board, and VCSE partners.
