Stoke-on-Trent City Council, together with MPFT, Whitfield PCN and VAST, are pleased to announce grants for VCSE organisations of between £1,000 and £10,000 from an overall fund of £60,000, administered by the City Council.

More About Ageing Well in Whitfield
Ageing Well Whitfield is all about helping people in our community feel good and stay active as we get older. Everyone gets older, but right now, people in the area spend almost ten fewer years in good health and are losing their independence earlier than people in other parts of the country. Health and care services are busy, and local charities and businesses have amazing ideas but often only short-term funding to make them happen.
Sometimes it’s hard to know where to turn; volunteers, social prescribers and community connectors have lots of people to see, and information doesn’t always get shared easily.
Living well isn’t just about services it’s about trust, confidence, feeling connected, and having chances to join in. We can help everyone to feel more in control of their wellbeing as they age through encouragement and reassurance.
That’s why we’re bringing people and services together in North Stoke, building trust, and supporting everyone have more choices to invest in their health and feel more in control. We hope by doing this more of us can stay active, feel less lonely, and enjoy life for longer better!
This work is initially a 1-year pilot, geographically based within Whitfield Primary Care Network (PCN), working with a wide collective of partners across local authority, NHS, educational institutions, and voluntary, community and social enterprise (VCSE) organisations.
Whitfield PCN has identified an initial cohort of people (271) living in the area who are likely to benefit from a targeted intervention to support their journey to ageing well. After an initial conversation with social prescribers, funding will enable VCSE organisations to work relationally with patients within their own communities, offering a continuing touchpoint for them to access a range of services relevant both to clinical need and broader social connection, while also providing partners with a continuing means of understanding a patient’s progression.
Grants for VCSE Organisations
We recognise that many VCSE organisations already deliver activity that supports people to age well in Whitfield but may not have the capacity to engage fully in the pilot, particularly with monitoring, reporting and evaluation processes.
This small-grants programme aims to enable VCSE organisations to take a relational approach to engaging in the pilot, building relationships with local people and other delivery partners. The grant may also be used to deliver new activity or increase service capacity as referrals from the programme increases.
It is our intention to work together to develop longer-term funding options, and to consider how to scale up the pilot to support other neighbourhoods across Stoke-on-Trent.
